Looking for a pediatrician? We're accepting new patients of all ages! Whether you're expecting a baby, new to New Jersey, or just want to switch providers, we'd love to meet you.

Our friendly staff can answer any questions you have about our practice. We even offer free meet-and-greets with our providers, either in person or virtually!

Transferring care? It's easy! We accept patients under 18, and for a smooth transition, we can request your child's medical records directly from their previous doctor. Alternatively, you can fill out our new patient forms online beforehand:

Want to schedule an appointment? We'd love to see you! New patients need to fill out the intake forms below before their first visit.

Expecting a little one? Congratulations! Call us 2-3 days after discharge to schedule your baby's first appointment. We'll register your newborn right then.

New Patient Checklist:

  • Insurance: Do we accept your child's insurance? Call us to see if we accept your child’s insurance.

  • Primary Care Physician (PCP): Have you chosen Dr. Grullon as your child's PCP on your HMO plan?

  • First Visit Essentials: Bring your current insurance card, immunization records, and any other relevant information (e.g., diagnoses). If transferring, bring a completed medical release form.

Expecting?

  • Find the Perfect Fit: We prioritize excellent care aligned with your family's values.

  • Insurance: Check our website to see if we participate with your plan.

  • Enroll Early: Call us after you decide to choose us. We'll need insurance information for both parents.

  • Prenatal Visit (Optional): Meet our doctors and get a feel for our office environment.

  • Let Everyone Know: Inform your OB-GYN and hospital that you've chosen us as your baby's pediatrician.

  • Schedule Your First Appointment: The hospital will likely recommend a follow-up visit within days of discharge.

  • Newborn Records: Before leaving the hospital, request that your baby's records are sent to us.

Important Note: Add your baby to your insurance plan within 30 days of birth to avoid any charges.
